Appointments

Office hours are by appointment only. Every effort will be made to accommodate emergency visits. Secretarial services for scheduling appointments are provided Monday through Friday from 9:00 am to 5:30 pm. All major credit cards, checks, or cash are acceptable forms of payment. Although we participate in many managed care plans, this information should be verified prior to scheduling your appointment. Patients enrolled in managed care are required to present appropriate referral forms at time of initial consultation. Provisions will be made for those patients with out of network benefits- please discuss with the office administration.

Cancelation Policy

If you are unable to keep an appointment, we ask that you give 24 hours of notice. This will enable another patient to use the available time. There will be a fee of $50 charged if less than 24 hours notice is given for cancellation.
